About 3 qts. A phone call while changing oil and oil sending unit, resulted in me forgetting to put OSU back into the engine. I started it up and out came 3 qts of oil in the blink of an eye. What a mess.
 
I had a road call this past week to a local chevy dealer ship. A car carrier was loading up a used Hummer H3 and blew out a hydraulic line. I got him going, but not before dumping 20gallons of hydraulic oil back in it. Guess who got to clean up the parking lot after he left? It took me 3 bags of floor dry and about 1 1/2 hours.

i work for a company that manufactures hydraulic axial piston pumps. one night someone shut down this 1 particular test stand, but the filter pump did not shut off. so oil kept getting pumped into this 1 tank, but there was nothing sucking it out. (when that tank gets a full a valve is supposed to open and gravity lets the oil fall into this other tank) the next morning when everyone came in there was approximately 150-200 gallons of hydraulic fluid (shell iso 46) on the floor. the problem with that test stand that caused all this was a known problem they just kept putting off and putting off. after that incident they fixed it immediately (imagine that). needless to say ALOT of people did ALOT of mopping that day. all the oil/mop water was recycled accordingly

now can anyone top that?? lol

About 8 quarts of Amsoil 10W30 due to a cross threaded filter housing. realized after the second attempt that I would need a new housing/nipple but by then had pumped out about 8 quarts of the expensive golden liquid.

Fortunately had a mat on the floor that caught most of the oil. Had to discard it though as contaminated.

I was emptying my Rangers oil on Friday for a 600 mile weekend trip and mis-placed my 15 quart pan so when the oil slowed its drip rate the oil was pouring over the handle and not in the pan. Not only did it spill a cup or two of oil ,but it got on the bottom of the drip pan and as i dragged it from under the truck it left 2 more oil circles.


That's a big mishap? That's SOP for me, for some reason.

About 4 quarts. Changing oil in my old CJ5 in a back alley and rushing to beat a snow storm. It was the first and last time I didn't check to see if the gasket had come off with the old filter. It didn't. Nice mess. That mistake will never happen again.
 
A couple quarts here and there. Nothing I can remember with a lot of specifics, so no big ones. I do remember a couple of coworkers through.
First was in Vegas. Another temp had put a street sweeper on the rack to do a full PM. I got there as the hydraulic reservoir finished emptying...on to the concrete. I dont remember exactly why he opened the drain valve without the tote under it, but he was not particularly bright anyway. When I got there his brain was still in the midst of a BSOD and reboot cycle. So I ran, grabbed some oildri, and started pouring a mountain around the puddle to keep it from spreading.
The second was at a WalMart. I watched a coworker get mad and push the rolling catch pan (maybe 10-15 gallons?) with enough force it jumped the stop at the end of the rail and dumped its contents.
