Very rough start in Honda insight

2006, 3 cylinder, 33,000 miles, 30 degrees

I started my Honda #2 for the first time since thanksgiving. The engine sounded really really rough. I was not happy at all, though it sounded "normal" after the temperature crawled to 4 bars. I've started plenty of cars in cold weather but they never sounded like this.

Maybe the oil drained back from the filter?

How is this filter mounted? Upside down or sideways?

Did you check the oil level?

Does the filter have a silicone (orange) anti-drainback valve? Are you using Honda filters?

Maybe the fuel was getting stale?

I'd make sure you have fuel stabilizer in the tank, engine oil to max mark, and a filter with a silicone ADBV installed in the future.
 
Probably had a valve lash adjuster or adjusters bleed down from setting that long. The engine will run rough when that happens. Had nothing to do with the oil. It takes a short while for them to pump up.
