With the drive to use more American made parts in their cars, it is likely that the ones involved are American made. While I suspect that both incidents are related to Toyota's switch to drive by wire technology (they weren't used to what they could and could not do), perhaps they could solve this issue by going back to 100% Japanese parts.
By the way, my mother's 2008 Toyota Avalon is affected, which makes me really happy, because the other day when I was at the gasoline station filling up her car for her, the engine would not start on me initially, but I thought it did, so I pressed the pedal. When I realized it wasn't started, I started it and the engine went to 8,000 RPM for 2 to 3 seconds. Thankfully, the parking brake was on (and I put the car into neutral while slamming the brake as soon as I realized what was happening), but it would have collided with another car if it had not been for that. I assume this issue is what caused it. Her car is going to the dealership ASAP.
