This one looks to be an Ecotec engine cartridge application. They are known for having post use 'smooshed' appearance. Here it produced angled pleating which resulted in a tear. This phenomenon has been seen in many cartridge brands posted here. Ironically, Protec cartridges in this application have had some good results. All that said, with unknown fci could be a factor too.

I was going on what owner of vehicle said as extra guard. This filter is actually a protec.
 
Terrible that it tore! If posters are indeed correct. I ran a Hengst on my Saturn L200 2.2L & never had any trouble. Just used the OEM plastic cap.
