I always rent the "economy", or subcompact class, or whatever is cheapest. Its a roll of the dice, but more often than not, I show up at the counter and they are all out of subcompacts and they give me something bigger for no additional charge. The best one was the Cadillac Deville I got from Hertz instead of the Focus or whatever box I had reserved. I was riding in style that day!
I think the only real problem I ever had with a rental was the Camry we rented in Chicago; it was supposed to be an "Illinois and adjoining states only" contract and in our infinite wisdom we took it to Kansas City MO (hey, Missouri is still an adjoining state to Illinois, right?). On a whim, I popped the hood and noticed some sort of misalignment on the accessory pulleys was tweaking the serpentine belt and it was frayed to nearly half its original width. Not wanting it to break on the road somewhere, we took it to the Kansas City airport and attempted a swap. They didn't want to give us one of their cars in exchange for what they considered to be a "Chicago" car! It took about an hour of conference calls with the agency in Chicago (who wanted to hang us for taking it to KC, but I had to convince them Missouri was an adjoining state) but eventually they gave us a different car and all was well.
