Which fast food places seem to care about being clean?

Agree, but I think the actual location is the biggest factor. If the restaurant is in a dumpy city (or dumpy area of a city) with lazy and entitled employees, it's more likely to be neglected. We recently visited a Raising Cane's in south St. Louis. The trash was overflowing and every single table was filthy. And most of the "employees" on duty were standing around playing with their phones. We had similar experiences at local Steak and Shakes, Wendy's, etc. Kind of gave up on fast food in the area because the rotten service and filth were everywhere.

Compare those experiences to a South Texas Whataburger: friendly and courteous employees and managers = a clean and pleasant experience where one may enjoy the best burger and service in the fast-food business.
 
My local Whataburger and Chick Fil A. Spotless, including the restrooms.

It really depends on the franchise owner and what kind of details that the manager of a location insists on. There's a Popeye's here that's impeccable. It's really weird too. The location used to be a KFC, but I'm not sure how that went down. The employees are mostly South Asian (not all though), although I remember when they opened it seemed more like typical fast food employee demographics around here. But overall the place is kept extremely clean.

Whataburger is Burger King. Just slower service and more expensive.

Also I’d pass on eating at any fast food joint in south St Louis. When I lived there about the only place I went was Ponderosa Steak House for the Sunday morning buffet or the IMOs on Shaw for their lunch special. That was the early 00s though I know the area has changed a lot since then. I worked at Soulard Farmers Market and it was quite sketchy back then. Now the whole neighborhood is filled with hipster studio apartments.

I was going to say this. I know someone who works there and they get internal inspections frequently

In-N-Out is 100% corporate owned. I’m thinking they send out an employee to check the condition of the dining room often to restock the condiments, tidy up the beverage fountains, and clean the tables/floor. However, I understand most of their business is drive-thru. I have been to a couple locations without drive-thrus.

I wonder if Tim Horton's (the parent company) will take over BK locations.

Looks like most of the locations are run by seven major franchise corporations...several of which are in real trouble. About four or five closed up in Metro East St Louis (where I used to live), with more closures coming.

Yes, I live here in Louisville too…
From the Taco Bell on New Cut that would not serve police , or the one in Hikes Point that is gross, list goes on…
Only one I ever liked was on Taylorsville rd, near Hurstbourne.
There are so many Taco and TexMex places here— the Bell doesn’t stand a chance.
Not a fan of El Nopal either
 
WOAH! Nearly 1,000 restaurants and I've never heard of Culvers! I simply must try one.
I'd expect some of the menu items to be "non-teenager" food.
One right down the street near me, they have a good pork chop/loin sandwich !!!! And cheese curds*
Just gotten too expensive lately.

As someone indicated, management and franchise ownership makes a difference towards cleanliness. Around here, Chic Fil A and Arby's seem to be the best.
