2003 Honda Accord 235k km Camshaft lobe wear. Could this be due to 0W-20 ?

My 03 CRV is the same, multiple lobes bad. I bumped up to 5w30 and that has slowed itdown. Plan to go until it starts misfiring, or I find a good deal on a cam.

I tried to get a local garage to quote changing the cam and lifters, but they wanted to do the whole head. I passed on it before getting a quote as I knew it’d be expensive.

Get a magnetic drain plug. Can't hurt. And you can "watch" any progression of symptoms (with great subjectivity and inaccuracy). Consider shorter OCIs since you'll have metal floating around more than usual.

Again, all this is predicated on the owner's unwillingness to fix it and a stated life span of 5 years (no criticism either way)

I open each filter now, so I “know” it has slowed down (not really, can’t see the small stuff—but no more big chunks!). I should shortened the change interval tho, the oil hasn’t been looking great lately. Fresher is probably better.

They are good cars if still in shape body wise compared to whatever else you can buy new or used with a big unknown. Time to gather parts when you don't need them straight away. A cam isn't a big deal to replace.
 
Honda had some metallurgy issues with their camshafts in the mid 2000’s. J series and early K24 were most known for it.
 
I don’t think you’ll have trouble getting a few more years out of it. This is a gradual process, and it will be pretty bad before you get misfires, and even then it can go a long time. If it were mine, I’d bump to a 10-30 oil to promote a sturdier film over the wear surface, but it’s really hard to know if that would have any value at all.

Also, since the mechanic didnt take any pictures, we really have no way of knowing the depth of problem here - it’s made it this far ok!
