8AM to 11PM five days a week is gonna kill you. Get rid of that job.

Make sure you're at the hospital when your Mom has that surgery.
 
Did your dad not pick you up on TriCare. They do have to cover you, if you pay for it. TriCare is $200/month for adult children, but it may be cheaper for you to pay that than for a month or two than pay the doctor and test bills.

You need to make a difficult choice keep pushing yourself with those insane hours including commuting or buck up and tell the boss it cannot work that way for you and have them work with you, if you are a valuable employee they will change those hours. If they won't you need to quit.
Once you have your health back you can once again get a stressful draining job back. But honestly folks doing that almost always die an early death.
 
If you do end up having to pay the medical, be sure to first call them and talk to them. Tell them the whole story, and then ask them for a discount. It is a coin toss / 50-50 chance, but many medical places have a huge mark up on the bill because they expect to get stuck with no pays. A no pay that is marked up is better for them than one that is not marked up. They can sell it to a collection agency for more, or use it as a bigger deduction (claim it as a loss) on their taxes.

If they know you are going to pay, they may discount it quite a bit. However it is easier to get these discounts if the bill is not over-due.
