NO, no, no! You don't need to cut anything to remove the lucite plastic towel bar from the end mounts. Look at the end of the bar closely and you will see that there is a rectangular "insert" inside the bar at both ends. The "insert" is made of clear flexible plastic similar to polyethylene (think plastic shampoo bottle) that is friction fitted inside the towel bar. Elderly customer wants me to remove the towel bar and install a grab bar. There is going to be significant damage to the drywall if it is adhered to the drywall. I will let my customer know before I try to remove it.
is it made of porcelain or ceramic? it looks like the old fashion kind that would have been grouted into ceramic tile work. maybe someone got creative and adhered it to the drywall/plaster

You’ll want studs to mount a grab bar if not possible I’ve used these togglers for grab bars tv mounts and even wall cabinets.

Lowe’s sells grab bars both w/ & w/o the trim ring.PicA grab bar would have a 3 - 4" trim ring at the wall so that should cover up any damage.
Look for two small recessed screws. Many times they are the same color as the bracket and not readily apparent. If it's not held on by double sided adhesive and you hit it with a mallet it can damage the drywall and the bracket.Check underneath for an allen screw, otherwise tap on the bottom with a mallet.
EDIT, yes if thats caulk , cut around it with a utility knife.
