Connections to hot water heater

More of a curiosity. The third pipe connection in picture. Not gravity recirculating or if it is then it does not work. Take 5 minutes for shower to get hot water.

This is in Italy. Water heater in bathroom about 7 feet off the ground, hung on wall.

The BX cable connection is the electricity.

Power comes in via the ribbed conduit (BX cable). Trav is referring to the 3/8" (approx) metal tubing that's smashed in the middle of it's visible run, that goes into the non-finished/covered hole in the wall.

There is this on the wall (picture,) a foot below the hot water heater. Maybe it pops out and sprays water down. But no idea what it really is. Could be a towel hook that comes out but I pushed on it to see if it pops out. Nothing

In the US the T&P relief is near the top of the tank as it's a T&P not just a P relief. With a T&P relief it cannot be in the cold water line.
