Blackstone sample kit question

This nifty kit includes a small cling sticker with spaces for miles/hours, date, and next sample due. Am I supposed to fill it out and stick it on the sample bottle? the instructions don't say what to do with it.

Another thing: on the paper slip we are supposed to disclose makeup oil, but sump size is not required?

I put mine in the inside top left hand side of my windshield to tell me when another UOA is due. If you want you can put the sump size in the remarks section and they can keep it on file.
