I have a 2008 Ford Ranger with 39K mikes. I drive about 4k a year and have been doing a full syn. change once a year. Should I change that to bi annual with a conventional or synthetic blend? Would a 0w20 be ok as she calls for 5w20?

If you live anywhere that does not have snow stay on the ground for months at a time:
I'd run the cheapest name brand 5w20 conventional thats on sale and change annually (4k) OR Full Synthetic 5w20 and change every two years. (8k)
If you live in part of America the beautiful that STAYS below freezing in winter:
Name brand 0w20 or 5w20 on sale and change annually.

Bi annual oil changes would be OK, IF you stick with a major brand (Mobil, Castrol, Shell/Penn or Liqui Moly) engine oil and use a real good filter (Motorcraft, Mobil, Amsoil or Castrol etc).
Not much difference between an 0w20 and a 5w20, but I would use the various oil company oil guide or finder pages of their web sites to see which oil they think you should use. Might well list an 0w30!
As you are already using a full synthetic, I would stick with that type of oil.

You might want to update your location to get more accurate recommendations in the future. Climate can play a role.

+1 and no need to use anything expensive.
Rural King's cheapest API oil would be fine for your use and vehicle as would Meijer's store brand.
OTOH, if you catch one of the M1 semi-annual MIR offers and combine it with a store deal, you can run the Mobil 1 Advanced Fuel Economy that tig recommends for less than the cost of store brand conventional.
This would be a good choice for those Jan and Feb Indiana mornings when you just have to start and drive this trucklet.

Here in South Texas , I run regular conventional SN oil in my dad's 1999 S10 with the 2.2L 4cyl.
Currently it has Formula Shell 10W30.
It never makes it over 3K miles in a year.
Currently at 18 months on this OCI. I'll change it at the end of the month.
I've been doing this the last three oil changes.
No issues.
But we don't have winters here like up north.

Disagree there. As long as it get to a high enough oil temp to burn off moisture, running a quality oil in a low mileage application can certainly go 2 years.

Most new German cars have the time only part of the OLM set to 2 years, BUT that is based on using an approved or recommended major brand HC synthetic, GTL base or group 4 synthoil.
If you use a cheap non major brand engine oil, I would change it every 6 months or 3000 miles max, so using a more expensive oil can save you money.

With synthetic you might very well be able to stretch it out longer. If you wanted to go 2 years, I'd take a couple of samples for UOA along the way (leaving the oil in).

In this application, the important thing is moisture control. If it gets to full OP temp every time and gets say 10~20 miles runs, OK on good old every day oil. If it runs short runs in the cold, you better rethink what you're doing.
I don't disagree with RK or TSC oils. They're fine. But if it's building up moisture and lots of cold start condensates, then I'd be changing 1/2 way through winter like Feb 1 and definitely staying once a year ...
