A rogue tire smashes a Rogue

I'm a semi mechanic at UPS. Being that New England Motor Freight folded years ago, I'm thinking that trailer is old and has Budd/stud pilot wheels, not modern hub pilot wheels. If you look at the video close, it appears the left front outer is missing on the NEMF trailer. I assume the tandem's aren't slid all the way rearward.

You gotta know what the heck you're doing working on Budd/stud pilot crap and rust has no business being present on anything. You have a stud on top of a stud with a nut and reverse threads the drivers side to be aware of.

In addition to the mechanic screwing up, I question the pre-trip inspection the driver is required to perform.

Wow, good catch! I'm not a semi mechanic but watching channels like busgreasemonkey & others it's crazy how many "tire shops" have no business touching a wheel! Add in a special design & it's guaranteed a loss. Glad no one was severely injured.
 
That's litereally the best case scenario for that rogue tire.
I've seen situations where the tire/wheel goes under the vehicle, or the tire/wheel goes into the passenger compartment.

Similar thing happened to a girl friend in college. She was headed to work driving I287 in NJ. A wheel came off something from the other direction and went right into the windshield of her car, just like that video. That afternoon, after the carcass of her car was hauled away and she got back home, she borrowed her mom's new Mercedes to go to class. Didn't realize she had a concussion from the morning thing, blacked out going through a red light. Thank God nobody was hurt, but the Benz was roasted. 2 nice cars in one day. She was a fun chick.
 
My old ems partner worked a crash similar but the tire went sideways through the windshield and killed this poor lady going to work. The tire actually bounced through the grass median from the opposite lane, sad. Glad this person was luckier.
