Other than what is noted the viscosity sticks out to me. If this is a 30wt oil then Id quess it mechanically sheared horribly or the fuel dilution test was errant and there is chemical shear. I guess it could be a 20wt from the factory.
Car is driven 25 miles one way each day typically, when its driven, taking between 20-30 minutes to destination. RPMs averaging probably about 3-4K. Warm ups until the coolant needle moves into the operating area and cool downs of 1.5 to 2.5 minutes depending on ambient temp.
