My brother asked me to replace the belt on his 06 Uplander 80k miles. he also said he wants the tensionser replaced. certainly it is easy enough - but is it necessary?

Yes, you should replace the tensioner and any rollers at the same time. The bearings can wear out, among other things.
I rarely repjace the tensioner.unless the spring is crunchy when moving the tensioner or has lost its tension I leave it alone. My jeep has 143k on its original tensioner but the wheel has been replaced. Im out west so I don't have to deal with salt getting in the spring
Agree with Chris....if the belt is not squealing from lack of tension, the spring action and bearings are smooth, then there's no reason to replace. I've only ever replaced when one of those conditions showed.

Something I encountered with my old V8-powered Audi S4 was that the belts were redesigned to be a bit shorter at some point (to prevent belts from getting thrown, which was a rare and annoying problem). This necessitated replacing the tensioner and roller as well.

Nope and not on a Toyota unless this part has a known failure rate. It is a very easy part to change out if it fails.
That being said I did not even bother having my water pump changed out on my wife's Subaru (now 155k) when exposed during a timing belt change.

I changed one at 200K miles, just because it had 200K miles on it. It was OK, but at that point, do you really trust it? It was around 15 years old at the time.
