My 93 E350 Ford Van engine will pulsate (rev up and down) at idle if the weather is hot and I have been ideling for awhile in traffic. Usually when this happens black smoke starts coming out the exhaust pretty heavily. In the winter this never happens. O2 sensor?
 
Try removing and cleaning the Idle air control valve. The older Fords were notorious for this problem. They get clogged up with carbon and will "possibly" cause this problem.

Good luck I have a similar problem, along with start stall issue I am trying to resolve.


It worked for my brother on a 93 F-150 4.9L, I wasn't as lucky.
