Toyota wheels fall off? Is this real?

Toyota offers BZ4X buyback because the wheels can fall off? But a dang Harbor Freight torque wrench for goodness sales!

So much for the big guys, I guess. And I love my Toyotas. Sheesh.

I guess my question is, how hard can it be to mount a wheel? Why are they falling off?
My 93 4WD PU strippie went half a million miles....

It's a really weird issue to have.

Is the problem with the studs? Nuts? The wheels themselves?

You'd think it would be as easy as a TSB that says "inspect, replace any damaged parts, and properly reinstall wheels."

I just can’t wrap my head around things coming loose that quickly if everything is torqued down. Yes, very weird indeed.

The only thing that makes sense to me is a material failure of the lug nut seats of the wheels.

If it was just studs or nuts you'd think it would be an off-the-shelf fix.
