As long as they're ASE certified, they'll directly pay the shop.

I have a hard time believing their going to cough up an $8k DSG at a dealer without batting an eye or wanting to hunt around for a used/poorly rebuilt one.

Many ext warranties will give the administrator the discretion to use used or rebuilt parts. This includes the Genuine Toyota Extra Care Warranty.
With that said, MaxCare is unbeatable for its price and terms on out-of-warranty german makes. The length of time is often up to 6 years from the Carmax date of purchase, not the manufacturer's in-service date!

Yes, that's a very unusual situation in aftermarket warranties.
