High Positive Fuel Trim -sometimes-

Check the valves. Out of adjustment valves cause all kinds of weird things also make sure the ground on intake isn't loose or hooked up to valve cover.
 
Injectors? More than one? Or just one o-ring?
The rubber seals at the tips of the injectors where they insert into the head were all (4) hard and brittle. The injectors just fell out loosely when I took off the fuel rail and with the new seals, the injectors had to be lightly pressed and squeezed in. The old ones were likely the original 13 year old seals and since the rail/injectors had been removed/reinstalled at least once without replacing the seals, my guess is that's why there was a slight vacuum leak there; they weren't mated with the head and were seeping air.
