99 tacoma 3.4 2wd. I have a behind the wheel drive test coming up. changed the fluid and shift seat bushing/seal and it improved (happens once in awhile.. have researched a rebuild (mechanic said shift plate plus a whole kit). If I can get through the behind the wheel test I'll live with the problem. any info appreciated.

Interesting first post. Ask the driving test evaluator if starting off in second gear is permissible given the mechanical condition of your vehicle.

Most transmissions that I’ve been around that pop out of gear do it on decel. Avoid running in 1st gear too long. Get it rolling and grab second.
