2021 Accord 2.0t 0w30 AFE 3100mi OCI

That's what he was driving at. Blackstone doesn't measure fuel dilution, they infer it from flashpoint and it is typically WAY off from a lab that uses GC. If Blackstone told you 1.5% you can safely assume that it is at least twice that, if not more.
I got you. It wouldnt surprise me by the smell of the fuel and how it feels between my fingers. That’s why I stopped using 87 octane as well.

Nuthin' wrong with that report on a 10,000 mile turbo engine. No problem with shearing. The oil starts at 8.6 and now it's 7.84 which is plenty of vis to keeps the parts separated. Pup 0w-20 feels thicker/smoother/more quiet than the AFE in my Camry. Try the Pup next; I like it better than the AFE. Ymmv

0w-30 AFE starts at 11.5 cSt @ 100C.

What if they claim "no fuel presence"?

Blackstone? It doesn't matter. The ASTM flash point test has relatively high reproducibility and repeatability to start with, then inferring some other parameter from that is increasing the possibility of error. A direct measurement always beats a derived value.
Has anyone ever questioned them directly on this method?

There was a 37% decrease in viscosity in 3,000 miles. I wouldn't be happy about that. Flashpoint looks relatively ok, so I'm not sure how much fuel is present. Often in B/S Labs reports flash point drops to mid 300's when a lot of fuel is present in the oil.

That is very concerning but wear metals are normal. I'm switching to a Euro oil and sticking with 30wt. I'm hoping to increase intervals at some point in this cars life.

That's a good point and something to consider. Sometimes viscosity changes don't seem to impact wear metals. Mobil uses a clean VII so I wouldn't be too concerned about leaving anything behind.

Starts at 8.6? That is WELL into 20 grade.

I’m not sure I believe much on that report, and I wouldn’t have certainty as to what the viscosity deviation is due to - if it exists at all.
It makes me wonder if the quality is going down? The more reading I do, the more I question BS.
