Just got this truck from my mother in law who drove it after my father in law passed. It runs well but could run better. The internals at least I can see through the fill hole look clean. Brakes need to be flushed and the power steering as well. My first change will restore and protect for at least 4 oil changes. It doesn’t burn any oil from the last time I checked it. Differential will get 75W140 and lsd additive. Plugs were done a few years ago. Anything else to look for? Running a bottle of redline SL1 as well.
 
Flush the cooling system, check the transmission fluid condition, depending on fluid condition and service history consider a drain/fill.

5.4 2v is a good engine aside from the risk of spark plugs blowing out if not torqued correctly.
 
Stopping and steering are the most important areas and I would concentrate on inspecting those systems before doing any fluid changes. I wouldn't be surprised if a brake overhaul and some front end parts are needed.
 
Ford updated the torque specs for the spark plugs on these engines to 28lbs to help prevent the blow out issue. I would go re-torque them to the new spec if it was my truck to make sure they were done right.
The plugs were done a few years ago. I’m assuming the 2 valve doesn’t have the long reach plugs?The plugs that can break.

The 2V gets the about-two-threads plugs that just blow out.

I'm the leader of the Dorman Sucks Club, but the 4R70 pan is nice for the drain plug. Have one on my wife's '08 and my '07 and my non-profit's '03 and have installed several on customers' vehicles.
 
How many miles? I’m at 181k on my ‘99 5.4 and aside from pretty miserable gas mileage it’s a great truck!
 
lol just saw that after I posted. whoops. Anyway should have plenty of life left in that truck. nice and easy to work on for the most part. I’ve got a P0401 code for EGR insufficient flow. I’ve got the parts just haven’t installed yet.
 
OP if your OD cancel quits it's the tiny wires on top of the column under the cluster. Happened to my non-profit's '03 at only 130k.

The story as I fabricate it is that Team TFI got a bunch of pacemaker wiring from a dumpster at a medical device company and figured it'd be perfect for the OD cancel switch. Once they had completed this faux pas it bought them time to continue designing their next HUGE FAIL for 2004 release: the 3V

No, they do not have the long reach plugs that break. As D60 mentioned they have the heads with small amount of threads in the heads for the plugs to screw into which makes tightening the plugs to the correct spec essential for preventing the plugs from blowing out.

115K. Today me and the my wife drove it down to a truck accessory place. They were awesome enough to take the camper shell off and dispose of it. Was worth trying to sell since the shell only fits step side. It drives great. Swapped out the PS fluid with Amsoil Red cap ATF. I also got some max life as well. Next up oil change. Going to try Valvoline R&P with a Fram Endurance filter. Trying to find the fuse for the power windows. Can’t seem to find that. Fixed the stuck rear door with a zip tie to support the cable…wouldn’t open. She’s coming along.
 
If you drop the pan and find the "plug ball" you know the trans hasn't been serviced before (which is not reason to panic anywhere other than BITOG)

Make sure 4wd engages reliably by doing tight turns in a dirt lot. The '03 trashed its CAD shift fork which is a not-that-common problem, but common enough Dorman offers a replacement. I just machined some brass bushings and attached them to the OEM fork

I still occasionally get frantic phone calls about that part and what it is and why it is it floating in the pan. I may or may not know multiple techs who would toss them back in the pan just to mess with people.

I might add, no anti seize on the threads.
