Did you prime the pad with product before polishing ? That may have caused it to come apart.

Yes, I did prime the pad. When doing some research I saw many complaints of these pads shredding. It seems to be caused by the Velcro on the backing plate digging in to the outer edge of the pads since they are the same circumference. For $5.99 I can always just buy another pad when I need one.
