Why?

Air in the ABS pump. I would flush it and it would come out perfectly clear for half a bottle, then activate the pump and a few ounces later pure foamy milk.
 
If you haven't already, I would replace all of the flexible brake hoses and start over with the brake fluid fill process. With enough age and a bit of corrosion, the reinforcing mesh inside the brake hoses can yield, allowing the hoses to bulge as the path of least resistance to the pressure applied by the master cylinder, instead of making the pads clamp against the rotors.
