Judging by the low miles, I would say the problem was dry starts rather than cold starts. I would guess it sat for periods of time before being driven again.
But, all considered, I don't think .014 wear on one cylinder is that excessive. Some engines had higher nickel content than others and who knows what quality of oil was run in it back then...
