P2122 and P2138 codes and "Accelerator system malfunction" Toyota Hybrid

I have 3 year old Toyota hybrid that has been modified with an electronically controlled left foot gas pedal. There's a switch that is pressed 3 times to activate the left foot pedal, otherwise it operates normally with the right pedal.
About a month after installation earlier this year, l began getting a Check Engine Light with the message "Accelerator system malfunction visit your dealer." Car does not drive if CEL light is on. If l wait about 10 minutes, the car starts fine without the CEL and warning message and it runs flawlessly. The issue was surfacing very infrequently in the beginning, but it now occurring 3 or 4 times a week. The shop that installed the left pedal carefully checked the installation and said everything was fine. Everytime l bring it to the shop, the car does not kick the CEL and they cannot diagnose it. This only occurs while starting the car. It never happened after driving it if it started without the CEL.
The shop that installed the left pedal wants me to take it there again instead of the Toyota dealer. I plugged my odb scanner in today and it spit out p2122 and p2138 from memory. That is "throttle position sensor".
Any ideas? It only occurs at startup. If CEL doesn't show at startup, car is good to go. Is it possible that the install of left gas pedal impacted something? I will take it into the installers shop again this week.
 
99.9% chance this is related to the left accelerator pedal. Any info on the make/model of this left pedal?
