Again by looking at that pipe no way scale is causing the blockage .
1. water flow not sufficient .
2. slope in drain again not sufficient.
3. kids guests using excess amounts of TP or throwing other sanitary items down drain.
when you see the puddle where is it exactly leaking from ? floor drain back up ?
 
The only thing that makes sense in the above post is number 3. Scale buildup in a will create a giant snag for anything solid to catch on. If a pipe is clogged, it generally stays that way. 45 degree fittings should be the only thing on a sewer line besides a pipe coupling.When we had teenage girls living with us, tampons were the cause of a few backups. Another thing that I've seen slow down water movement is tree roots in the pipe. They will come out on the cleaning head of a snake if they are in there. They get in the pipe when the tree is looking for a water source. It only takes a small crack in the sewer line for it to get in the pipe, and start growing. And the tree or bush can be a ways away from the sewer line.,,

The plumber said slope and flow were OK, both the first guy that came to clear the clog and the resin coating company said that. They measured slope with their borescope tool somehow and had me run a bunch of water from the shower to check the flow. They said the pipe did shallow out a bit near the clog but still had acceptable slope and water flow.

I guess it’s possible my wife has been flushing some kind of product down the toilet and not telling me or using something in that shower that’s causing clogs but she doesn’t admit to it if so. We haven’t really had any guests since Christmas and it didn’t clog while they were here. Last time it clogged she said she had taken a #2 and had to plunge it to force it down. I tried to get details on how the TP was managed but couldn’t get that info.

The”leak” is coming from the garage floor drain when it clogs, the drain backs up and sewage puddles around it. We marked the clogging spot on the floor and the drain is about 20 ft back from the spot where it clogs.
 
I looked closely at the pic of the pipe again I know its isolated to that view but I see no signs of grease build up in the pipe at all and honestly I believe that scale is not enough to clause things to stick unless water flow is not sufficient .
for ie my parents house has 80 plus year old cast looks similar to scale in your house the last time I had the clean out opened we checked for flow had my father load the toilet with a lot of toilet paper far more then you would normally use .
Had him flush while I watched and flow was quick and waste pushed by at a good clip.
so I believe the flow rate of the toilet may be insufficient .
I would tell everyone to double flush after finishing there business and trial this to see if it helps any !
truly believe you do not have to reline the cast !

Try to avoid those grainy, facial "scrub" cleaners----it used to have literal sand; but they switched to cellulose. Only the lab knows how dissolvable it is---mix it w/oil, paper, you got something like putty.

It's like a reverse rolling snowball---a little bit of stuff forms a bump on the pipe....then that bump gets bigger and never washed away 100%.
 
I also have an old cast iron sewer line under concrete that I have been dealing with for 40 years – my thoughts & input for you:

-> I think you can successfully use it as-is. You have professional feedback that the pipe is intact, has no roots, and it is sloped acceptably. The scale is an unfortunate nuisance (my pipe looks like yours), but periodic maintenance has helped me avoid back-ups.

-> For maintenance, about 6 years ago I found some rather low-cost pressure washer accessories on Amazon that greatly help:
- 50-ft 3/8” hose, >3000psi.
- A nozzle with various orifices: one that sprays forward, and 4 that spray radially at 45 degrees backward, and spins the front of the nozzle. It pulls itself through such that you have to restrain it, and cleans very well!
- A lighted fiber-optic that I taped onto the hose. I have impressive video of the action!
- Long Gloves!
Caution – The pressure washer is for periodic maintenance - not for blockages. If you trap high-pressure water it is going to splash right back on you – trust my bad experience!

-> I also have low-flow toilets. As a precaution, I use multiple flushes when there is a big load.

-> If this is a main line, also consider that the kitchen waste can also greatly contribute to blockages. Oil and grease will ‘freeze’ on the cold cast iron. The build-up is impressive and hard to remove. I have learned to greatly limit food products going down the drain, and use cold water to rinse dishes off.

-> Your floor drain puts you in an unfortunate situation, but it gives you access to the sewer which is critically important! An idea to eliminate the back-up is to find a floor drain insert that has a built-in check-valve to keep sewer water and gases within the pipe.
Q: Do you even need a floor drain? If not, then you can simply cap it with an expandable insert that sits flush with the floor. These are available at Lowe’s / Home Depot for about $8, and you maintain easy access.
 
Find yourself an old school 3+ gallon toilet for that bathroom. You could also install a specialty macerating toilet, but I wouldn't want the hassle and maintenance headache.
 
I'd look to see if a plumbing supply store carries any products, or has any contacts that can give you a second option or product to use.
