Sometimes you don't. The rattling works if the ball clogs and sticks, but the valve can fail in other ways. Best to change at a mileage marker and continue with that interval. Look for blow by and valve cover leaks. The PCV hose and grommet can need attention as well as the valve.
The spring behind the ball inside (if equipped) can become week overtime from the heat in the engine. I would just make it a regular replacement item when you do your spark-plugs. It does so much and can cause quite the headache if not working properly. It's super cheap to replace on most vehicles anyways...

A couple of thoughts:
Have you looked at your owner's manual, if you plan on keeping your car a long time then I would change it once a year or every 2 years.
I would not buy a cheap 1, it is best to get 1 from the dealer as the cheap ones are no good. A bad PCV can cause so many problems that it is cheaper to replace it more often.
Some people may think that I am crazy to change mine on my El Camino every year, but I have 365,000 miles on my small block chevy so I must be doing something right.
When a PCV gets clogged it can lead to:
1) Oil Leaks
2) Crud in your engine
3) Engine Wear
I do not believe in shaking the PCV to see if it works because I cannot tell if the spring inside is going weak.
