Highway driving - Cabin noise and hearing protection

Unless we're talking some super knarly tires or other mods to a modern day vehicle, or of course, driving with the windows down, I'm surprised noise is still an issue?

Any vehicle I've owned is very quiet with the windows up at highway speeds and this was a mix of cars, pickups, SUVs and minivans.

The only time I had a situation where the vehicle was absolutely deafening was when a wheel bearing went out on my 2008 Santa Fe on my way to work and I had to drive it 40 miles home that way. I had to use earplugs. I wear ear plugs most of the time for my job that is insanely noisy in places.
 
My Mustang is the loudest riding car I've owned with the road noise being horrible. It takes the fun out of driving the car and is why it mostly just sits. It must have no sound barrier at all inside of it. Very irritating to ride in on long trips. My fiancee's 07 Mustang GT with basically the same body style and chassis has no road noise. I can't figure it out.
