Yes and no. We have a yearly training on it but don’t have to sign our lives away and get reminded that they will fire us if we violate it. Didn’t have to sign an employee handbook and be reminded that Iowa is an at will state either.

Also called business conduct guidelines. A lot of it is common sense, doing the right thing. Don't accept bribes and don't offer bribes even if it's common n the country you are doing business in.
More than one. We go through probably 3 of them every year with trick questions etc. Basically we want to make sure if someone at work screw up, the company will not be sued even if you sell a nuke warhead design to Russia for a crazy night with Natalia and Olga.

Well, we are bound to hold ourselves to the highest ethical and moral standards, both inside and outside of work, as representatives of.....
Don't sell to a company you think may sell to Iran or North Korea if things look fishy
Don't try to make fun of people who are different in any way (religion, orientation, background, believe, look, etc)
Don't try to cover up financial or legal mistakes
Don't forgive someone because they are friends of boss, don't try to forgive a vendor or customer because they have forgiven you in the past
If someone related to your boss or your coworker is doing business as customer or vendor, go through legal department for review
Don't bribe government officials
Don't accept gifts above a "small amount" if you suspect they gift because you have the decision making power
Don't have sex with coworkers you manage directly or indirectly
Casting couch is not allowed.
Don't retaliate if someone piss you off, let your management deal with it
Report your boss if he or she is doing something fishy
Be nice to coworkers.
It is not racism or discrimination if someone is being a jerk to everyone, don't abuse the reporting mechanism.
That's about it. Most of the time I screw up the questions later because I report too many things that are actually allowed in the guidelines (i.e. sleeping with someone else's boss or direct report, accepting free lunches if not expensive, gifting gifts that are small amounts to non gov employees).

Don't take bribes
Don't load a truck faster because of a gift or influence
Don't drive like a jerk especially if people can identify the Company we work for
Don't be a jerk to a vendor, contractor or driver or client
Don't take merchandise, this is especially important in our Foreign Trade Zone......
Don't display weapons in company parking lot even if State law says it is OK
My first 2 jobs, yes I had to sign one. My current job working for the county, no. Wonder why we have problems.

We agree to it during Orientation and it is touched on during videos. It is for the life of employment with the Company, not just yearly.

Yep, it is in a big pile of documents we are required to (e-)sign every January. Required for our state certification and all that jazz.
Sometimes you have to bend the rules to be employee of the month,..,
