Large employers are self insured and use the insurance company as the administrator/mediator. The rules of coverage are always being tightened and modified to harm the employee to lower expense to the employer. The 4 large corporations I worked for all had this model. Pretty free and easy in the 1980's. Like a knee on the neck in the 2020's.
