Yeah, VLC gets it for sure. Easy and (practically) all-inclusive.
I've had 2 situations in the last several years where Mplayer handled something that VLC couldn't. Like VLC, Mplayer works on Windows, Mac and Linux; although it's nowhere near as easy to get and install.

VLC has pretty much every format covered along with every codec.

Definitely VLC, as stated repeatedly above. The interface is horrible, and some features I really appreciate are lacking, half-implemented, or broken, but it plays more often and more reliably than anything else out there. It's also easy to install and use.
Being cross-platform, the VLC developers made VLC "skin-able"; meaning you can get and apply many different interfaces.

VLC here, too. Have it on three machines, set as default player.

I don't know if it does or does not; but there is a very handy-dandy Firefox extension called VideoDownloadHelper that'll sniff out and download darn near any embedded media you can think of.
