He's probably aware of the basic problem, but knows that it's not going to get fixed. Let's say your basic problem is overall cardiac output and the solution requires open heart surgery so that they can use that laser thingie that punches a bunch of micro holes in the muscle wall to reverse/stop heart disease. Now we're into $150k+ of work and a whole lot of risk for a situation that's not imminently life threatening.
You may not be in bad enough shape to fix yet. I know, it's a real odd way to look at it.
