I realize that it looks like the accident in the video appears to have been caused by a spacer or wheel bearing, but I did have a scare with loose lug nuts about 25 years ago.
While on a family vacation to Washington DC, I had axle bearings fail on my Suburban. I left the Suburban at a shop that was recommended by a friend's mother, who lived in Maryland, just outside of DC. We drove a rental home to South Carolina, and I flew back to DC when the Suburban had been repaired. I paid for the repair, got in my Suburban and started working my way home on I-95.
It wasn't too far before I started feeling that the truck just wasn't tracking quite right, so I pulled off at a rest stop to check it out. I found that all of the lug nuts on the LR wheel were loose. And not just a little. It took several turns to tighten them all up. I remember feeling sick, thinking that the lug nuts could have worked loose enough as I was driving, to let the wheel fall off.
I was glad it didn't happen. But now, I check the wheels before driving away, whenever someone else has worked on the wheels of my cars.
