Are the lifestyles of people with lots, and large tattoos, the same as those with no tattoos?
Excellent question. By looking at different outcomes between twins, this study is trying to minimize those differences. Twins presumably have similar upbringing and genetics and likely fewer lifestyle differences.

Not just twins, identical twins. Twins are like siblings who just happen to be born the same day. Identical twins have identical DNA.

The theory is that the embryo separates into two (probably at the 2 cell stage) resulting in 2 babies having absolutely identical DNA.

One interesting finding from identical twin studies: Identical twin studies in Finland established that degenerative disc disease is almost purely genetic. Identical twins having discordant occupations (for example one a heavy laborer, one an accountant) will have strikingly similar spine x-rays at the same age.
