Alight, time to tap into the BITOG knowledge base. Have my MIL's old Stihl BG50 handheld blower and it basically just stopped running last time I used it and wouldn't start back up. Did some troubleshooting and slapped a new carb on since storage practices were definitely in question. Seems to have spark, fuel and compression but still doesn't start... It will actually flood out if I pull the cord a LOT of times but clearly lights up my spark tester and compression is at 120psi.

Obviously I'm missing something here but I don't get.

Pull the wire mesh spark arrestor screen out of the muffler and throw it in the trash. I'll bet it is all plugged up with carbon deposits. However, if you need it to legally operate it on federally owned forest lands, you can burn off the carbon/oil deposits with a propane torch.

If not already done, get fresh new gas and mix it with a 40:1 or 50:1 gas to oil ratio using a premium full synthetic 2 cycle oil like VP Racing or ECHO Red Armor.

Yeah did all the usual stuff like lines, filter, spark mesh
Even though it tests good on your spark tester, I'd put in a new NGK spark plug and verify the proper electrode gap. Quick squirt of ether starting fluid and if you don't hear a pop when you pull the rope, you aren't getting spark.
