pulsation in brakes

So it seems some say pulsation come from uneven pads deposits from improper break-in. And others say it comes from uneven wear of the rotor due to runout.

When it comes to improper break-in are we talking only performance cars or do we also include the daily driver? I would guess most people never break in their brakes, nor are the told to.
 
Yes I did get new rotors. I indexed old rotors and new ones and they were about the same. The hub was causing the same high and low spots. I re-used brake pads.
