Multiple crew failures and wind shear led to January crash of B-1 bomber, Air Force says

Zoinks! Two of the crew failed the toxicology screen. That's gonna leave a mark on their file.
Numerous issues found during the investigation. Part of the runway technology was out, and the tower appears not to have informed the crew, which if the crew knew might have reduced the likeliness of the crash.

Maintenance crew. Amazing how many people say, “well, my state made this legal” without realizing it is still illegal for duties involving aircraft maintenance or flying.

This wasn’t “windshear”. It was “moderate wind shift”.

My simulator ride as part of my interview at United Airlines included multiple shifts of greater magnitude than this.

A competent pilot should have been able to control airspeed, and keep the airplane both on speed and on glideslope.

This pilot failed to do either.

A competent instructor should have spoken up, used standard communication, and directed a go around when the flight parameters exceeded SOP limits.

This instructor failed to do any of that.

$450 million gone because a culture of ineptitude and sloppiness existed in this command.

I’m just glad there were no fatalities.

Actually, there were fatalities, in a manner of speaking.

More than one person involved in this accident will see their career bite the dust.
