Because it is not nearly as effective. I tried it once, for appearance's sake, and found it very unsatisfactory.

If you had nearly perfectly balanced wheels and tires that were also somehow perfectly match mounted, you might get away with it. But if they were that good, they wouldn't need to be balanced at all.

It also increases the second harmonics, so it is not as well balanced as not doing it.
 
So, another thought I had was that if the left to right balance (dynamic, correct?) was off and you had to add a weight- then what if that threw the rotational balance (static?) apart?

Depends.

If you lost a weight, then both the static and the dynamic balance would be off - and replacing that weight would bring both back in balance.

However, if the tire was statically balanced, but had a large enough dynamic imbalance, you need to add 2 weights to fix - OR - you dynamically balance the tire and the machine will tell you where to put the 2 weights to achieve both static and dynamic balance. If you don't remove the weights before the balance procedure, you'll wind up with 4 weights on the wheel - but it only takes 2 weights to both statically and dynamically balance a tire.
