I've actually heard of this happening with those manifold pre-cat systems.
I would expect whatever mechanical matrix that holds the platinum in place would have to also fail... which will not have a 1:1 correlation to your trouble code. IE it could fail without a code or you can have the code without failure.

A cat's platinum plated, latticed ceramic matrix composite can melt or crack and crumble. The bits and pieces may blow out the rear end until the cat is literally hollow. There are also metal matrix cats.
