Ford Emission problems

My parents '02 Escape with the 3.0L has been showing a P1131 and P0401 for some time now. I've wrestled with the problem on and off to no solution. The facts I know right now is that the Bank 1 Sensor 1 O2 reads 0 volts at idle or driving, the was obtained from Torque Pro. All other sensors read ~0.6 volts operating like they should be. We have already replaced that B1S1 once. It didn't change the problem.

For the P0401 I have replaced the vacuum lines between the egr valve, solenoid, and intake. If there is insufficient egr flow would that mean the egr valve is closed and if so would the egr tube be cool to the touch after running for 15 minutes? I'm having "writer's block" with this problem. Any insight would be appreciated.
 
For the p0401 code. Did you use an egr solenoild from ford? I went through several aftermarket ones. One from the dealer fixed and lasted.

alot of fords have a problem with the DPFE sensor failing, these are very high failure parts. As far as 0 volts on front sensor is the car really lean like a vacumn leak at idle, iwould sure check.

I have replaced the dealt pressure feed back sensor about three years ago. It was and O'Reilly's off brand, so that might be the case. The only think we have replace as of recent was the B1S1 O2 sensor and vacuum lines. I might go ahead, order a motorcraft sensor. Should check the continuity of the delta pressure feed back sensor wiring harness and the solenoid wiring harness.

O'reilly Masterpro engine management parts are not the best imho. I would get either a Motorcraft or BWD sensor.

I'm starting to think that way too. I took the upper intake off a while ago to do the spark plugs and used fel pro gaskets. I might just order some new motorcraft gaskets and check a vacuum line that runs under the intake.
 
When you replace a DPFE and still get a 400 code, pull the throttle body. More often than not, the passageways to the EGR are clogged. I have cleaned out these passageways on Vulcans and Duratecs with great success. A throttle body is easy to pull on those once you have all the intake plumbing out of the way.
 
The upper intake is only a 18 months on the car, I'll check though. I plan on taking the upper intake off and going over it. The bank 1 sensor 2 was reading in the high side at 0.86-0.89 volts at idle but bank 2 sensor to was reading around 0.69-.72. I think that would be a pretty big difference between the two (actually 4). I;m thinking there's a leak on that back bank somewhere.
