The methodology seems to just look at crash fatalities per 100k and ignores everything else.
Traffic density and congestion probably play a part, too.Roadway design and maintenance affects the crash fatality rate. There are definitely differences in how roads are designed and maintained from place to place.

That's the same way I feel, every time I read one of these reports on worst drivers, and don't see Boston listed in the top 10.I don't see Miami anywhere on the list so I don't believe it.
