2003 Ford Econoline wagon with front end vibration

This is a 145,000 mile truck with what seems like two separate issues.
1) Front end vibration when stepping on the brakes above 20 mph. It's not a pedal pulsation so doubt it's warped or excessive rotor runout.
2) Front end vibration most noticeable at 62-67 mph although it comes and goes at lower speeds as well. No shake felt in the steering wheel.

What to check? Suspension bushings? Inner and outer tie rods? Wheel balance?
 
Worn shocks, bad alignment, bad wheel baring, bad wheel balancing, wheel weights could have fallen off, even a worn spot (budge) in the tire thats sometimes goes unnoticed.
