Any idea what oil filter they were using? On mine it had some generic workshop filter ("mighty something" brand) so maybe stretching intervals combined with a filter that doesn't filter well enough can do this. That could explain "how did it happen even with Amsoil".

Always calls for a lubricated bolt, because it gives a more consistent clamping force. Torque measurements are an assumption that the friction on the threads is achieving the clamping force which is why angle measurements are way more accurate, because then you know the amount of bolt stretch...
I got a different number than OP did. I was maxing out a 250ft-lbs torque wrench and still not at 60 degrees with a new bolt and properly oil-lubricated threads and top surface of the washer.
I don't have the exact number because I couldn't measure that high but I'd say it was closer to 280...
Always possible. It's a legit tender though: Noco Genius10 that I did put on the AGM setting. Right now it's "full" and holding 13.9V which should be on the money for AGM float voltage.
