Since I'm wanting to get my UOA journey started, I settled on Polaris in Indianapolis because they are local to me (although with the USPS in Indianapolis being famously awful, it might take longer than getting something else).
Since Polaris is geared heavily towards large industrial accounts, it is a bit of a hassle to get it done, but it's completely doable for a home gamer. You'll set up an account with them and add your "equipment items" (i.e. your vehicles) to that account. They'll want everything from the kind and micron rating of filtration to your sump capacity. You can also add your oil and its viscosity grade (it's all in their database).
For their premium analysis with particle counts, you're looking at buying at 10-pack for $334 or so. I'll prepay for the analysis but not for the shipping since it will take me awhile to burn through 10 samples.
