A) My Ranger was 15 years old w/ I can tell you that Ford assembly line filters are made from hacksaw resistant stainless steel.
The media was a dull yellow color. I have no idea what it started as.
There were black crumbs throughout the media. The glue holding the media was black.

B) A fuel filter from an old Toyota I had was full of water at ~100K. I smashed it open and felt it wasn't gasoline.
C) I've cleaned enough small engine gas tanks of weird, marshmellowy goo to conclude filtration is a good idea.
That's all I know of the subject....oh, and that the fuel filter is the most overlooked. I heard that so I don't really know.
