I'm gonna guess that the valve stem failure was caused by valve float. Maybe the valve spring was at the low end of tolerance for spring pressure.

Personally, I'd want more zinc for those old flat tappet camshafts. Shell Rotella T5 15w-40 has relatively high zinc content.
