    Clay bar alone should be sufficient. Cleaner wax is not a good idea on a windshield. You can also try one of the Nanoskin AutoScrub sponges which are supposedly even better than clay bar (they're similar to the Magic Eraser)...

    Try to run chkdsk on it. Could just be something stupid like a damaged file table or whatever. If you plug it into your old PC, is it readable? Are other USB devices readable? Alternatively, try booting to a Linux USB key and see if you can read it there.

    Most of them can be washed. Check with the manufacturer. It's typically done on the delicate cycle, cold water, with a detergent that does NOT have fabric softener. You may want to do a second rinse cycle to ensure all of the soap gets out or it could leave a weird film on the paint.
