Root cause found. The cam lobe failed. The valve was adjusted per Honda's spec but that adjustment was done on the other side of the lobe in the valve-closed position, and the cam is good in that position. In the open position the failed exhaust valve wasn't fully opening by a significant amount.
Here they are. These plugs were in the engine for 96k km (60k miles). The plug from cylinder 6 has some accumulation that doesn't appear on 4 and 5 plugs. Also there is a short across the plug 6 gap, which I assume is debris from when the valve disintegrated.
I'm am leaning toward such a conclusion myself - i.e., that is an isolated random failure related to degredation of the valve itself. But I lack the experience to be sure about that conclusion.
Unfortunatley I didn't record that information before disconnecting the battery. Rookie error. I can say that van had no drivability issues before the sudden cylinder six misfire.
