If your coworker was seeing that kind of failure, it was clearly either a poor install (heads weren't re-machined, cheap gasket kits, amateur install). I suppose it's possible that a block slipped past the QC controls at the plant, but I suspect this story itself is BS. I have NEVER, EVER heard...
